Job summary

Monogram Health Professional Services is seeking compassionate Advanced Practice Providers (Nurse Practitioners or Physician Assistants) to deliver high-quality in-home care to patients with multiple chronic conditions in the United States, including diabetes, hypertension and CKD.

You will work within an integrated multidisciplinary team, providing long-term patient relationships, continuity of care at patients' homes, and telehealth options.

Qualifications

• Active and unrestricted RN/NP/PA license.
• Current/unrestricted DEA certificate or eligibility to obtain.
• Minimum of two (2) years of direct patient care experience.
• Field-based role requiring travel to patient homes, reliable transportation, valid driver’s license, and auto insurance.

Responsibilities

• Provide in-home, evidence-based care that reduces avoidable hospitalizations and improves quality of life.
• Perform initial evaluations, wellness exams, follow-ups, acute visits, and post-discharge visits.
• Educate patients and caregivers on disease management, preventive care, medications, and testing.
• Participate in recurring virtual integrated care team meetings.
• Engage in audits and documentation reviews to ensure compliance with regulations and best practices.
• May practice in SNFs, ALFs, hospitals, and other settings as applicable.
• Perform point-of-care testing (A1C, Spirometry), prescribe medications, order tests, and coordinate with Monogram physicians.
• Coordinate care with PCPs, specialists, and ancillary services.
• Complete documentation accurately and timely.
