Job summary

Monogram Health is seeking a skilled Nurse Practitioner or Physician Assistant to provide compassionate medical care mainly for patients with chronic kidney disease. This remote position allows professionals to work autonomously, conducting assessments and coordinating care for complex patients.

The ideal candidate will possess an active RN and NP/PA license, board certification, and experience in direct patient care, while contributing to improved health outcomes and quality of life.

Qualifications

• 2+ years of direct patient care experience required.
• Ability to work autonomously and communicate effectively.
• Knowledge of HIPAA and OSHA regulations.

Responsibilities

• Conduct assessments including wellness exams at home and virtually.
• Deliver evidence-based care to reduce hospitalizations.
• Coordinate care with primary providers and support patient needs.
