Job summary

Harrisonburg-Rockingham Community Services Board (HRCSB) is seeking an ACT Nurse (RN or LPN) to provide in-home services to clients with severe and persistent mental illness in Harrisonburg, VA. You will administer medications, assess side effects, educate patients, and work with the ACT Team for comprehensive care.

The role requires a rotating schedule including day, evening, weekend, overnight on-call coverage.

Qualifications

• Must hold an RN or LPN license in Virginia and be in good standing.
• Experience in mental health and/or substance use disorders preferred.
• In-home nursing care including medication administration and education.

Responsibilities

• Administer and deliver medications in clients' homes.
• Assess medication side effects and monitor responses.
• Educate clients about medications and symptom management.
• Coordinate with ACT Team for comprehensive care planning.
• Maintain client records and ensure confidentiality.
• Participate in day/evening/weekend/on-call rotation as required.
