Job summary

Elk Hill Farm, Inc. is seeking a Residential Case Manager to support service planning and documentation for youth in state-licensed residential care.

You will coordinate with therapists, guardians, and treatment teams to develop goals, service plans, and discharge planning aligned with licensing standards. The role emphasizes case management, documentation accuracy, and collaboration with families, referring agencies, and program staff.

Qualifications

• Build and maintain positive relationships with youth and families.
• Maintain Medicaid/licensing compliant service and discharge plans.
• Coordinate with treatment teams to ensure compliant documentation.

Responsibilities

• Support service planning and documentation to enhance program services.
• Advocate for the rights of youth and coordinate with families and guardians.
• Develop and update behavior support plans and progress reports.
• Assist with transportation, scheduling, and compliance with licensing standards.
• Maintain accurate medical and case management records for each youth.
