Lincoln County Health and Human Services Department seeks a Wraparound Case Manager (CM2/CM3) to provide case management services within the Child & Family program. The role entails care coordination, advocacy, and supporting clients with understanding mental health disorders and symptom management skills.

CM2/CM3 levels offer hourly pay with different ranges; Oregon Pay Equity considerations apply. Oregon Driver's License and QMHP-related certifications are noted as expectations.

Qualifikationen

• CM2 requires a Bachelor's in behavioral science or an Associate's with 2 years of behavioral health experience or a high school diploma with 5 years in behavioral health.
• CM3 requires a Bachelor's in behavioral science.
• QMHA certification or equivalent within specified timeline is expected per MHACBO guidelines.

Aufgaben

• Conduct intake screens and assessments related to skill building or case management services.
• Perform assessment and treatment planning under supervision of a QMHP.
• Assist in individual, family, or group therapy and co-facilitate psychoeducation groups as appropriate.
• Provide care coordination, advocacy, and referrals, with symptom management education for clients.
• Complete records and reports per agency standards, Oregon Administrative Rules, and contracting agencies.
• Provide clients with aftercare planning and follow-up services.
• Collect laboratory samples as indicated.
