Merakey invites you to join our Blended Case Manager- Transition Team in Cambria County, PA. You will develop and coordinate services for clients, acting as the main contact for up to 30 individuals and coordinating with other providers to meet client needs.

Qualifications

• Bachelor's degree in sociology, social welfare, psychology, gerontology, anthropology, or related fields is preferred.
• Registered nurse licensure is acceptable.
• High school diploma with 12 credits in sociology or related field plus two years of direct contact with mental health consumers is acceptable.

Responsibilities

• Develop, coordinate, and implement services per the service plan.
• Serve as primary contact for no more than 30 individuals.
• Provide case-management, service planning, and coordination with other providers.
• Provide supportive counseling and participate in on-call duties.

Education

Bachelor's degree in social sciences

Registered nurse
