Job summary

Merakey is seeking a community-based Blended Case Manager in Colmar, PA to serve clients in Montgomery County. The role encompasses case management, service planning, and coordination with other providers, with a focus on increasing community stability.

The position requires managing a 25–30 client caseload, conducting assessments and service plans, and completing required paperwork for OMHSAS, Magellan, and local authorities.

Qualifications

• Bachelor's degree in sociology, social welfare, psychology, gerontology, anthropology, criminal justice, theology, nursing, counseling, or education; or RN; or a high school diploma with related coursework and experience; or high school diploma with 5 years of MH direct care experience.

Responsibilities

• Link consumers with resources and teach them to advocate for themselves.
• Complete assessments, service plans, and environmental matrices to determine level of care and contact frequency.
• Complete OMHSAS, Magellan, Montgomery County, and Merakey paperwork and be monitored for timely completion.
• Hold appointments with clients in homes, community sites, and hospitals; be familiar with Montgomery County resources.
• Maintain a caseload of 25–30 clients and meet with clients every 14 or 30 days per matrix, per productivity standards.
• Attend monthly supervision and weekly team meetings.
• Work typical Monday–Friday, 9:00am–5:00pm; occasional on-call based on client needs.
