Black Hills Special Services Cooperative (BHSSC) in Sturgis, SD seeks a dedicated Support Coordinator to be the primary contact for individuals receiving developmental disability services. You will manage person-centered planning, coordinate services with families, legal representatives, and healthcare providers, and ensure accurate HIPAA-compliant records across Therap and agency systems.

Qualifications

• Bachelor's degree in human services or related field.
• Strong verbal and written communication skills.
• Knowledge of person-centered services, rights, behavior supports, and regulatory requirements.
• Ability to pass a background check.
• Physical ability to lift 50 lbs.

Responsabilités

• Manage an assigned caseload and serve as the primary contact for individuals receiving developmental disability services.
• Coordinate person-centered planning and support services with individuals, families, legal representatives, and team members.
• Monitor ISPs to ensure goals and services are delivered and updated as needed.
• Maintain accurate documentation in Therap and other agency systems with HIPAA/Medicaid compliance.
• Promote individual rights, informed choice and due process, including rights assessments and rights restoration when applicable.
• Respond to complaints and service issues, ensuring timely resolution per BHSSC policies.
• Support Positive Behavior Support Plans and coordinate across interdisciplinary teams.
• Monitor incidents, risks, and safety concerns with appropriate reporting and follow-up.
• Facilitate communication across service teams and maintain relationships with participants, families, staff, and partners.
• Ensure regulatory compliance and support continuous quality improvement.
