Job summary

NLC Support in New Jersey seeks an Experienced Support Coordinator (Full-Time) for Bergen/Passaic Counties. This hybrid role requires field visits and travel within the counties; bilingual is preferred.

You will assess service needs, develop the NJISP, and coordinate with clients, families, programs, and providers; establish goals and service eligibility; develop the PCPT and monitor clients monthly.

Qualifications

• One year experience working with adults with developmental disabilities.
• Excellent written and verbal communication.
• Flexibility to attend meetings during the day and travel within counties.
• Hybrid role requiring field visits and travel within NJ.

Responsibilities

• Assess service needs and develop the New Jersey Individualized Service Plan (NJISP).
• Establish achievable goals, outcomes, and service eligibility.
• Develop the PCPT as a discovery tool for person-centered planning.
• Ongoing monthly monitoring of clients.
• Coordinate and facilitate team meetings at residential or day programs.
• Manage a caseload of 10–30 consumers; reside in New Jersey.
