Job summary

Qualifications

• Bachelor's degree in human services or related field and three years of related experience.
• Experience coordinating services for individuals with intellectual and developmental disabilities preferred.
• Knowledge of HCBS waivers, CCC Plus Waiver, and Medicaid preferred.

• Assess eligibility and needs of individuals.
• Interview individuals and families.
