Job summary

Encompass Community Supports in Culpeper, VA, is hiring a Support Coordinator I – Developmental Disabilities. This full-time role requires in-person visits to clients in home and community settings, linking them to services and monitoring outcomes.

  • Any combination of college education and experience providing services to people with DD, equivalent to 5 years.
  • Possession of knowledge, skills and abilities necessary to perform the essential functions of the position.

Responsibilities

  • Facilitates, assesses and evaluates clients’ need for services at intake and/or for people receiving active case management. Presents options and services based upon a needs assessment.
  • Develops an individual support plan based on assessments appropriate to the evaluation with the client/family consensus. Reviews and updates support plans when applicable.
  • Refers, links, and coordinates at intake or for people receiving active support coordination.
  • Prepares and maintains Electronic Health Records to document all activities performed with or on behalf of people receiving services.
  • Prepares applications, referrals for people to gain benefits or resources.
  • Advocates for clients’ human rights and follow program guidelines to meet regulatory compliance.
  • For people on the DD Waiver waitlist – complete waitlist updates such as VIDES, SARFs, priority checklist, and critical needs.
  • Provides resources for crisis intervention and prevention services.
  • Attends meetings, staffing, and conferences as they relate to client, staff and program needs.

SUPPORT COORDINATOR I - DEVELOPMENTAL DISABILITIES

Salary Range: $50,000 - $55,000

FLSA Status: Non-Exempt

Encompass Community Supports, formerly known as Rappahannock Rapidan Community Services (RRCS) is hiring a Support Coordinator I – Developmental Disabilities. Encompass Community Supports is a Virginia State community services organization that improves the quality of life in Culpeper, Fauquier, Madison, Orange, and Rappahannock Counties by providing comprehensive mental health, substance use, developmental disability, housing, children’s, and aging services. At Encompass Community Supports, our full-time employees work 37.5 hours weekly. Our Support Coordinator I – Developmental Disabilities team engages with individuals who have intellectual and other developmental disabilities to assess what supports would aid the person in living the life they desire. Support coordinators link the person to various services and resources, monitor the effectiveness of the services and coordinate among a support team. The SC I position can provide active Support Coordination only to people with Developmental Disabilities. Intakes can be conducted for people with Intellectual Disabilities and/or Developmental Disabilities. This position requires in person visits with individuals receiving services in home and community-based settings.

EDUCATION AND EXPERIENCE:

Required:

  • Education minimum: High School degree or equivalent.
  • Any combination of college education and experience providing services to people with DD, equivalent to 5 years.
  • Possession of knowledge, skills and abilities necessary to perform the essential functions of the position.

Preferred:

  • Bi-lingual – fluent in English and Spanish, written and oral.

PHYSICAL REQUIREMENTS:

  • This work requires little or no physical effort or exertion.

SPECIAL REQUIREMENTS:

  • Attends training and maintains certifications as required by the Agency and regulatory guidelines, such as CPR, First Aid, Person-Centered Plan, and DBHDS Case Management Modules.
