Job summary

The Warren County Developmental Disabilities Board is seeking a qualified and compassionate Support Coordinator to help individuals with developmental disabilities access services and supports. This office-based role emphasizes person-centered planning and coordination across agencies.

You will develop plans, advocate for needed services, document progress, and collaborate with families and providers. Reliable transportation for home visits is required.

Qualifications

• Bachelor’s degree in social work, psychology, special education, or a related field is required.
• At least two years of professional experience assessing service/support needs and developing or implementing individualized support plans for persons with developmental disabilities.

Responsibilities

• Develop and monitor person centered support plans.
• Advocate for individuals to obtain needed services and supports.
• Coordinate and monitor the delivery of services.
• Maintain accurate documentation in compliance with state and federal regulations.
• Assist in the development of new services and supports as needed.
• Collaborate with families, providers, and community agencies/providers.
• Maintain flexible work hours to accommodate the schedules of individuals and families served.
• The job is an office-based position.
