Job summary

Heartland Alliance Health is seeking a Manager of Housing and Health Integration to lead two program areas, supervise staff, and ensure high-quality care for people experiencing homelessness and health challenges. The role requires a clinical license in Illinois, a Master's in a related field, and at least 3 years in case management with supervisory experience.

The position is full-time at 4730 N Winthrop Ave, Chicago, IL, with standard hours and a No Wrong Door approach to care.

Qualifications

• Master's degree in a clinical field and IL licensure.
• Active, unrestricted IL clinical license (LCSW/LCPC).
• 3+ years in case management, care coordination, or social services.
• At least 1 year in a supervisory role.
• Experience with homelessness, chronic homelessness, or serious mental illness preferred.
• Experience with HUD-funded housing programs and/or HIV/AIDS services preferred.
• Strong trauma-informed care, harm reduction, and low-barrier approaches.
• Proficient with EHR and case management systems (CaseWorthy, Athena Practice, HMIS).
• Excellent clinical, communication, and interpersonal skills.
• Ability to manage multiple priorities and maintain documentation.
• Commitment to equity, cultural humility, and person-centered care.

Responsibilities

• Provide leadership and supervision to CBC program staff.
• Coordinate and ensure program service delivery aligns with standards and funder contracts.
• Monitor compliance, training, and performance against objectives.
• Oversee health screenings, referrals, and linkage to care.
• Maintain accurate documentation in CaseWorthy and Athena Practice.
• Assist onboarding and professional development of staff.
• Maintain a low-barrier, trauma-informed environment.
• Represent CBC in internal and external meetings as assigned.
