Job summary

Continuum Rehab Group is seeking a Speech Language Pathologist CF or CCC to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities in Waco, Texas. You will evaluate and treat patients to improve intelligibility, communication, and swallowing safety while coordinating with a multidisciplinary team.

Qualifications

• Must hold SLP licensure in Texas or eligibility to obtain licensure.
• CF/CCC eligible or CCC-A preferred for credentialing pathway.
• Experience in skilled nursing or long-term care settings is desirable.

Responsibilities

• Evaluate and treat patients with speech, language, cognitive, and swallowing deficits.
• Develop and implement short- and long-term treatment plans with goals.
• Document progress, prepare plans of care, and communicate with physicians.
• Collaborate with OT/PT and other clinicians on patient care plans.
• Provide family education and participate in case management and discharge planning.
• Maintain compliance with company policies and professional standards.
