Job summary

Continuum Rehab Group provides in-house therapy with Skilled Nursing facilities in Texas, New Mexico, Indiana, Arizona, and Missouri. We are seeking a licensed SLP to evaluate and treat patients, focusing on intelligibility and communication, and to support swallowing safety through targeted therapy.

The role involves direct patient care, treatment planning, documentation, and collaboration with a multidisciplinary team to ensure high-quality rehab services in SNFs.

Qualifications

• Evaluation and treatment within the scope of an SLP.
• Focus on intelligibility and communication improvements.

Responsibilities

• Direct all aspects of patient care including evaluation, treatment planning, goal setting, and documentation.
• Provide treatment for speech, language, hearing, and cognitive disorders.
• Develop and monitor treatment plans and progress notes.
• Collaborate with the healthcare team on individual patient programs.
• Complete Plan of Care and related clinical documentation.
