Wellstar is seeking a Speech Language Pathologist to evaluate patients and develop comprehensive speech-language plans of care. You will collaborate with physicians across the continuum of care, aiming for cost-conscious, quality-focused, and patient-centered rehabilitation services.

You will establish plans of care with patients and families, implement therapy, document progress, and ensure discharge instructions and follow-up resources are provided in line with regulatory standards.

Qualifications

• Graduate from an accredited speech-language pathology program.
• Licensed as a Speech-Language Pathologist (SLP).
• BLS certification or Instructor’s BLS required.

Responsabilités

• Assess patients and develop speech-language plans of care with physicians.
• Create and adjust treatment plans for speech, language, voice, cognition and swallowing.
• Establish discharge plans in collaboration with patients and families.
• Document evaluations and plans of care per departmental guidelines.
• Educate patients and families; coordinate referrals for rehab services.
