Job summary

Aristacare-Health-Services is seeking an in-house Speech Language Pathologist to provide services within our Skilled Nursing Facility in Plainfield, NJ. The role involves evaluating residents, developing and implementing treatment plans, and ensuring CMS-compliant documentation.

The position offers full-time and PRN options with competitive pay and comprehensive benefits. Active state licensure and ongoing continuing education are required to maintain credentials.

Qualifications

• Bachelor of Science Degree in Speech Language Pathology from an accredited program.
• Must have an active Speech Language Pathologist license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.
• CMS NetHealth EMR documentation experience or ability to document in CMS-compliant systems.

Responsibilities

• Provide Speech Language Pathologist services to residents in accordance with established standards of practice and department procedures.
• Adhere to documentation and billing procedures for Speech Language Pathologist services.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Conduct screening of residents to determine need for intervention/treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals and methods to achieve goals.
• Interpret and communicate evaluation findings and recommendations to residents, families, and the interdisciplinary team.
• Complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems.
