Qualifications

• NJ LSW/CSW/LCSW license required.
• 1–2 years healthcare setting experience, preferably skilled nursing/long-term care or rehab.
• Experience with discharge planning, Medicaid/Medicare, and MLTS preferred.
• Understanding of CMS regulations and DOH survey process preferred.
• Strong communication, documentation, and organizational skills.
• Ability to work independently while collaborating with an interdisciplinary team.

Responsibilities

• Coordinate admissions, discharges, and transition planning.
• Provide emotional support and advocacy for residents and families.
• Participate in interdisciplinary care plan meetings.
• Educate residents and families regarding Advanced Directives.
• Maintain accurate documentation in compliance with state and federal regulations.
• Assist with discharge planning, community resources, and post-acute coordination.
• Support residents through rehabilitation stays and long-term care placement.
