Job summary

Complete Care at Madison is seeking an experienced Social Worker (LSW/CSW/LCSW) to join our skilled nursing and rehab team. The role focuses on admissions, discharge planning, family support, and coordinating care across an interdisciplinary team.

You will support residents and families through transitions, educate on Advanced Directives, and ensure documentation compliance while working in a collaborative, resident-centered environment.

Qualifications

• Current New Jersey LSW, CSW, or LCSW license required.
• Minimum 1–2 years of experience in a healthcare setting, preferably skilled nursing, long-term care, or rehab.
• Experience with discharge planning, Medicaid/Medicare, and MLTS preferred.
• Understanding CMS regulations and Department of Health survey process preferred.

Responsibilities

• Coordinate admissions, discharges, and transition planning
• Provide emotional support and advocacy for residents and families
• Participate in interdisciplinary care plan meetings
• Educate residents and families regarding Advanced Directives
• Maintain accurate documentation in compliance with state and federal regulations
• Assist with discharge planning, community resources, and post-acute coordination
• Support residents through rehabilitation stays and long-term care placement
