Qualifications

• Licensure with the state of Alabama as a Social Worker required.
• Comparable combination of education and experience may substitute for the above requirements.

Responsibilities

• Assist patients/families in discharge planning and transition to the next level of care.
• Collaborate with physicians, UR staff, and other healthcare team members to transition patients to appropriate level of care.
• Ensure confidentiality and compliance with hospital policies.
• Document discharge plans and assessments in the medical record/departments database.
• Participate in Performance Improvement activities and mandatory department requirements.
