North Alabama Medical Center in Florence, AL is seeking a Social Worker to join our team. The role focuses on assessing patient needs and coordinating services to support patients and families within a compassionate care environment.

The position is full-time, reports to the Director of Case Management, and requires a Bachelor's degree with Alabama social work licensure within six months and Handle With Care certification within thirty days.

Qualifications

• Education: Bachelor's degree required.
• Licenses: Board Certified Social Worker in Alabama within 6 months of hire.
• Certifications: Handle With Care within 30 days of hire.
• Must be able to work in a stressful environment with appropriate action.

Responsibilities

• Interviews and assesses patients and/or family, caregivers, and legal representatives.
• Determines, prioritizes, and arranges necessary internal and external services.
• Participates in case reviews to evaluate progress with the healthcare team.
• Assists with discharge planning and identifies resources for patient needs.
• Provides individual, family and group therapy; supervises staff and interns.
