Job summary

RHA Health Services, LLC in Jackson, TN is seeking a Social Worker/Case Manager/Qualified Professional (QP) to coordinate person-centered services for individuals with IDD and mental health challenges within an Intermediate Care Facility.

You will lead admissions, develop PCPs, oversee service delivery, and ensure occupancy while collaborating with the IDT, guardians, and staff. This role emphasizes independence, well-being, and regulatory compliance.

Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology, Special Education, or a related field (Master’s degree preferred).
• At least 2 years of experience (in North Carolina), 1 year of experience (in Tennessee) working with individuals with intellectual and developmental disabilities; 1 year required if Master’s degree is held.
• Demonstrated experience in developing, writing, and implementing Person-Centered Plans.
• Working knowledge of ICF regulations, person-centered planning principles, and applicable Medicaid/HCBS rules.
• Excellent communication, organizational, and documentation skills.
• Valid driver’s license, auto insurance, and reliable transportation.
• Must be at least 18 years of age.

Responsibilities

• Lead admissions and census management, including pre-admission screenings and PCP upon admission.
• Coordinate IDT meetings and translate individual needs into PCP goals.
• Monitor PCP fidelity and progress toward outcomes.
• Conduct site visits to evaluate care quality and safety.
• Communicate with families and guardians to inform service planning.
• Ensure timely, compliant documentation per Medicaid/ICF requirements.
• Participate in training, audits, and team meetings.
