HonorHealth’s Shea Medical Center in Scottsdale, AZ is seeking a Case Manager Social Worker to coordinate patient care from admission through discharge, ensuring a smooth transition, high-quality outcomes, and effective collaboration with patients, families, and the health care team. Requirements include a Master’s in Social Work and active licensure (LSW/LCSW/LMSW).

Prior case management experience is preferred.

Qualificações

• Masters in Social Work Required
• 1 year as a Licensed Social Worker, and/or successful completion of health related field placement in Master's level Social Work Program Required
• 2 years in case management Preferred
• 3 years as a Licensed Social Worker Preferred
• Other Experience in management of behavioral health patients Preferred

Responsabilidades

• Accountable for an assigned caseload, works collaboratively with patients, caregivers, healthcare providers, and external partners to ensure that care is coordinated across the health care continuum and a smooth discharge
• Collaborates with patients/caregivers, hospitalists, specialists, and PCP to communicate plan of care and financial obligations
• Supports discharge planning, community resources, and goal setting to optimize outcomes and cost-effectiveness
• Advocates for patients, ensures appropriate level of care, and contributes to Case Management metrics
