Qualifications

• Undergraduate degree or equivalent experience.
• BSW with 1+ years of clinical healthcare experience or relevant internship.
• Driver's license and access to reliable transportation.

Responsibilities

• Establish a safe, effective psychosocial care plan for each member.
• Complete a comprehensive psychosocial assessment and develop an action plan.
• Collaborate with members, families, and healthcare providers.
• Identify referrals to financial, psychosocial, community, and state services.
• Advocate for members and navigate the healthcare system.
• Address age-specific communication needs with diverse members and families.
• Identify high-risk members requiring social services and intervene.
• Maintain records of plans, interventions, and outcomes.
• Participate in interdisciplinary team meetings and share knowledge.
• Understand insurance products, benefits, coverage limitations, and regulations.
• Handle PHI in compliance with HIPAA standards.
• Perform additional duties; in person at Long Beach SNFs 3–4 times weekly.
• Four 10-hour shifts (8:00-18:30) or five 8-hour shifts (Mon–Fri).

Education

Undergraduate degree or equivalent experience
