Job summary

Optum, part of UnitedHealth Group, is seeking a Health Social Worker/Case Manager in the Long Beach area. You will perform in-person care management at 3-4 skilled nursing facilities weekly, delivering four 10-hour shifts (8:00–18:30) or five 8-hour days while coordinating with members and healthcare teams.

Responsibilities include comprehensive assessments, action planning, referrals to support services, and HIPAA-compliant PHI handling to improve health outcomes.

Qualifications

• Undergraduate degree or equivalent experience.
• BSW from CSWE-accredited program or related social work degree.
• Driver's license and reliable transportation.

Responsibilities

• Utilize vetted resources to establish a safe, effective care management plan.
• Complete comprehensive psychosocial assessments and develop member-specific action plans.
• Collaborate with members, families, and healthcare providers.
• Identify and initiate referrals to financial, psychosocial, community, and state services.
• Advocate for members and assist navigation of the healthcare system.
• Maintain records of treatment plans, interventions, and outcomes.
• Participate in interdisciplinary team meetings and act as a resource.

Education
