Job summary

UnitedHealth Group's Optum is seeking a social care manager to perform psychosocial assessments, develop member-specific action plans, and coordinate with families/faculty providers in the Los Angeles metro area.

You will advocate for members, navigate the healthcare system, and ensure HIPAA compliance while engaging with diverse populations. In-person shifts at LA facilities are required.

Qualifications

• Undergraduate degree or equivalent experience required.
• BSW with 1+ year clinical experience in healthcare preferred.
• Driver's license and reliable transportation required.

Responsibilities

• Establish a safe, effective care management plan using vetted resources.
• Complete psychosocial assessments and develop member-specific action plans.
• Collaborate with members, families and healthcare providers.
• Identify and initiate referrals to financial, psychosocial, community, and state services.
• Advocate for members and help navigate the healthcare system.
• Maintain records of plans, interventions, and outcomes.
• Participate in interdisciplinary team meetings.
• Act as a resource to teammates and providers for complex cases.
• Understand insurance products, benefits, coverage limits, and regulations as they apply to the plan.
• Uphold HIPAA standards for PHI handling.
• Work in person at 3-4 skilled nursing facilities in the LA metro area with varied shifts.
