Job summary

Smith Mountain Lake Health & Rehab Center, a 90-bed skilled nursing facility near the Smith Mountain Lake community, is hiring a Social Services professional to oversee resident psychosocial well-being. You will coordinate planning, implementation, and evaluation of social services in accordance with federal, state, and local guidelines.

The role requires a Bachelor’s in social work or related field (or current state license), plus at least one year of supervised social work in healthcare.

Qualifications

• Bachelor’s degree in social work or related field, or active Social Worker license as required by regulations.
• Minimum one year supervised social work experience in healthcare.
• Knowledge of regulations governing activity services in nursing facilities.
• Basic computer skills and ability to use electronic health record software.
• Long-term care experience preferred.

Responsibilities

• Ensure the Social Services Department meets residents’ psychosocial needs.
• Plan, organize, implement, evaluate, and direct the department’s operations.
• Design an ongoing social service program aligned with assessments and regulations.
