Midlothian Health & Rehab Center is seeking a Social Services professional to oversee resident psychosocial needs within a skilled nursing environment. You will coordinate the Social Services Department, ensure regulatory compliance, and support residents through assessment-driven plans.

Requirements include a Bachelor’s degree in social work or related field (or a state-licensed Social Worker), plus minimum one year of supervised healthcare social work experience.

Competenze

• Bachelor’s degree in social work or related human services field, or valid Social Work license if required by state.
• Minimum of 1 year supervised social work experience in a healthcare setting.
• Knowledge of regulations governing activity services in nursing facilities.
• Basic computer skills and ability to navigate electronic health record software.

• Oversee the Social Services Department to meet residents' psychosocial needs in accordance with federal, state, and local standards.
• Plan, organize, implement, evaluate, and direct social services operations within the facility.
• Ensure ongoing social service program aligns with comprehensive assessments and residents' well-being.
