Job summary

Livingston Hills Nursing & Rehabilitation Center seeks an experienced Director of Social Work to lead psychosocial care for residents and support families through care transitions. This leadership role focuses on improving quality of life and coordinating social work services within a skilled nursing setting.

You will oversee psychosocial assessments, care planning, documentation, and discharge planning, acting as a liaison among residents, families, and the interdisciplinary care team.

Qualifications

• Strong communication, organizational, and interpersonal skills.
• Healthcare experience required; skilled nursing experience preferred.
• Social Work licensure preferred.

Responsibilities

• Conduct psychosocial assessments for new admissions.
• Develop and implement individualized care plans.
• Coordinate interdisciplinary care planning and discharge planning.
• Complete timely documentation, including admissions, quarterly assessments, and MDS.
• Investigate and document resident concerns and grievances.
• Serve as a liaison between residents, families, and the care team.
