Job summary

Concierge Home Care in Melbourne, FL is seeking a Clinical Team Support (Scheduling Coordinator) to help a busy home health team run smoothly. You will schedule patient visits, coordinate clinician calendars, and triage scheduling changes to ensure patients receive timely care.

Ideal candidates have home health scheduling experience, strong organizational and communication skills, and enjoy working in a fast-paced, team-driven environment; AHCA Level 2 background screening may be required.

Qualifications

Responsibilities

• Scheduling patient visits in accordance with physician orders and care plans.
• Coordinating clinician schedules to optimize productivity and coverage.
• Managing daily scheduling changes, call-outs, and urgent visit needs.
• Answering and triaging incoming calls to support clinical operations.
• Tracking medical supplies and supporting RAP/EOE processes.
• Maintaining accurate scheduling and documentation within the EMR.
• Supporting the clinical team with administrative tasks as needed.
