Job summary

True Care is seeking a Home Care Scheduling Coordinator to efficiently coordinate caregiver placement and visit confirmations for patients. This on-site role supports families and caregivers, ensuring accurate schedules and timely communication.

The successful candidate will manage a patient caseload, participate in trainings, and assist with coverage while maintaining confidentiality and high service standards in a fast-paced home health setting.

Qualifications

• Fluency in Mandarin and Cantonese is required.
• High school diploma or GED required.
• Experience in customer service is preferred.
• Excellent interpersonal and communication skills.
• Ability to multitask in a fast-paced environment.

Responsibilities

• Coordinate caregiver placement and visit confirmations between Home Health Aides and patients.
• Create and maintain staff schedules, confirm attendance, and communicate with caregivers, patients, and families.
• Manage caseloads and participate in trainings to provide high-quality service.
• Assist Team Lead with coverage and staffing challenges.
• Communicate scheduling updates and maintain documentation in HHAeXchange.
