Job summary

Franciscan Health Munster Campus seeks a Patient Care Coordinator (RN) to support nursing excellence in a clinical setting. The PCC collaborates with the clinical manager, applies the nursing process, assesses patient conditions, and initiates appropriate actions to meet needs.

Requirements include active RN license in Indiana, BSN for external candidates and internal candidates must obtain within 3 years; minimum 3 years nursing/patient care experience; BLS and ACLS; 1 year specialty preferred.

Qualifications

• RN license to practice in the state of Indiana or NLC equivalent.
• BSN required for external candidates; internal candidates must obtain BSN within 3 years of promotion.
• Minimum 3 years of Nursing/Patient Care experience.
• Basic Life Support (BLS) certification required; ACLS preferred or required per unit.
• Unit-specific certifications may be required depending on the unit.

Responsibilities

• Collaborates with the clinical manager to plan, coordinate and evaluate patient care activities.
• Applies the nursing process and assesses patients to determine appropriate nursing actions.
• Oversees unit duties, participates in rounding, and supports service recovery initiatives.
• May provide direct patient care when staffing requires it and communicates effectively with patients and families.
