Qualifications

• English language proficiency required.
• Graduate of a nursing program accredited by ACEN or CCNE.
• Current, full, active, and unrestricted RN license in the United States.
• Bridge program route to licensure may be considered.
• Temporary appointment possible with licensure pursuit (GNT exception).
• VA Handbook 5005 Appendix G6 applies to qualifications.
• Foreign education requires degree equivalency validation.
• Licensed in any US state or the District of Columbia.

Responsibilities

• Manage chronic diseases as part of the PACT RN Care Manager role.
• Triage patient concerns in person or by telephone.
• Apply PACT principles and advanced clinic access in daily work.
• Demonstrate nursing process: assessment, diagnosis, planning, implementation, evaluation.
• Collaborate in interdisciplinary practice to deliver holistic care.
• Promote health, disease prevention, and management of acute/chronic illness.
• Participate in VA total rewards and leave programs; schedule Mon–Fri.
• Telework: Not available; on-site position in Green Bay.
