Qualifications

• Current RN license in Vermont is required.

Responsibilities

• Coordinate the development and implementation of MDS assessments per Medicare/Medicaid regulations.
• Conduct skilled care assessments and develop care plans with the interdisciplinary team.
• Maintain accurate and timely documentation of patient information, including MDS and care plans.
• Collaborate with healthcare providers to ensure continuity of care.
• Identify and address issues related to patient care or MDS assessments.
• Participate in quality improvement initiatives to enhance patient care.
