CVS Health is seeking a dedicated RN Case Manager to assess, plan, implement, and coordinate case management activities for members. You will evaluate medical needs, improve overall wellness, and facilitate transitions to CVS Health programs.

The role emphasizes holistic care, regulatory compliance, and collaboration with a multidisciplinary team. Requirements include active RN licensure, 3+ years of clinical practice, and strong communication skills.

Qualifications

• Registered Nurse with active and unrestricted RN licensure in their state of residence, preferably with multi-state/compact privileges. Must have the ability to be licensed in all non-compact states
• 3+ years clinical practice experience required

Responsabilités

• Assessments conducted telephonically and face-to-face to evaluate medical needs and coordinate care.
• Develops plans to enhance member wellness and facilitate transitions to appropriate CVS/Allied programs.
• Uses clinical judgment to reduce risk factors and barriers affecting care planning.
• Reviews prior claims to assess potential impact on case management and eligibility.
• Evaluates member work capacity and related restrictions/needs.
• Collaborates with supervisors and teams for case conferences to optimize outcomes.
