Job summary

The role involves coordinating service plans, training staff, and ensuring regulatory compliance. Travel may be required as needed, with a focus on delivering high-quality, person-centered supports.

Qualifications

• High School diploma or equivalent required or preferred.
• Minimum 3 years related work experience are preferred.
• Current driver’s license, car registration and auto insurance if transporting clients.
• All state required training and certifications completed in mandated timeframes.

Responsibilities

• Assists in accessing medical and health-related services for individuals.
• Coordinates and evaluates plan implementation for services.
• Recruits, screens, hires and evaluates staff; manages schedules as needed.
• Implements and contributes to development and revision of programs and behavior plans.
• Schedules and provides training and recreational activities considering preference and age.
• Maintains client and staff reporting records in regulatory standards.
• Provides direct care in assigned programs weekly as scheduled.
• Performs other related duties as required.
