Job summary

Renown Health is seeking a Discharge Planning Assistant to support the Hospital Care Management and Acute Rehab teams. You will coordinate discharge plans, communicate with care teams, and help move patients timely to their next setting.

Daily rounds, referrals to home health, infusion, wound care, DME, SNF, hospice, and transportation arrangements are part of the role. Strong clerical skills and familiarity with medical terminology are preferred.

Qualifications

• Two years clerical experience in the medical field preferred.
• Understanding of medical terminology preferred.
• Working-level English language skills required.

Responsibilities

• Support RN Case Manager and Social Workers in discharge planning coordination.
• Attend unit rounds daily to communicate progress on discharge plans.
• Initiate referrals and arrange home care, infusion, wound care, DME, SNF, hospice as needed.
• Document all discharge planning activities in the medical record.
