Job summary

Oxleas NHS Foundation Trust is seeking a Registered Mental Health Nurse to provide specialist mental health care to offenders within a prison setting. You will work as part of the Mental Health In-Reach Team, delivering evidence-based interventions and contributing to CPA-driven care plans.

The role requires robust assessment, care coordination and collaboration with community and NHS providers to promote continuity of care and better outcomes for patients in custody.

Qualifications

• Registered Mental Health Nurse with NMC registration.
• Experience delivering mental health care in custodial settings.
• Ability to work within CPA care planning and risk assessment processes.
• Experience coordinating care with multi-disciplinary teams.

Responsibilities

• Provide specialist mental healthcare to offenders as part of the Mental Health In-Reach Team.
• Carry out assessment, screening and interventions for offenders with complex needs.
• Contribute to care planning, risk assessment and CPA processes.
• Facilitate individual and group interventions as required.
• Support transitions of care and continuity with community services.
