Riverstone Post Acute in Kansas seeks a dedicated Physical Therapist to provide rehabilitative therapy in a skilled nursing facility. You will assess cognitive and ADL functioning, plan care, and deliver interventions to improve patient outcomes, documenting progress according to regulations.

Experience in SNF is preferred; the role may involve supervision of PT aides and collaboration with the rehabilitation team to ensure optimal patient care and discharge planning.

Qualifications

• Bachelor's degree or higher in Physical Therapy or related field.
• Active state license required.
• 2+ years of experience in a skilled nursing facility environment.

Responsabilités

• Screen and evaluate patients with cognitive/ADL disorders; develop care plans.
• Provide rehabilitative, skilled treatment per standards.
• Administer ADL retraining, therapeutic exercises, modalities.
• Monitor progress and adjust treatment; document outcomes.
• Discharge summaries and maintain records per policy.
• Supervise PT assistants and aides when applicable.
• Communicate progress with supervisor and health team.
