Job summary

Carle Health is seeking a Licensed Physical Therapist to evaluate and treat patients across all ages in our rehabilitation services. The role requires designing comprehensive plans of care, documenting progress, and adapting treatment based on patient outcomes.

The PT will educate patients and families, perform examinations to determine diagnoses, and collaborate with the medical team to ensure optimal discharge planning. Supervisory duties for staff may be included where applicable.

Qualifications

• Must hold a current Licensed Physical Therapist (PT) license.
• BLS certification within 30 days of hire.
• Completion of required continuing education related to acute stroke care.

Responsibilities

• Evaluates and treats patients of all ages, developing and adjusting plan of care based on patient response.
• Educates patients and families using effective teaching methods.
• Performs examinations and clinical assessments to determine diagnoses and outcomes.
• Documents care in accordance with policy and regulations.
• Collaborates with the care team and supervises PTA and rehab technicians as required.
