Job summary

Wellstar Health System in Georgia is seeking a licensed physical therapist to assess, plan, and deliver rehabilitation services that improve mobility, relieve pain, increase strength and prevent deformity across the patient care continuum.

You will collaborate with physicians, families and staff, document evaluations and plans of care, provide education, discharge instructions and follow-up resources, and ensure compliance with regulatory standards and Wellstar policies.

Qualifications

• Doctorate Physical Therapy or Masters Physical Therapy or Bachelors Physical Therapy
• All certifications are required upon hire unless otherwise stated. PT - Physical Therapist BLS (Basic Life Support) certification may be required
• Effective communication skills, customer service focused, and team oriented with basic computer skills

Responsibilities

• Implements the physical therapy treatment plan
• Selects appropriate treatment activities to progress patient towards goals
• Revises goals/plan of care with patient/family input
• Provides discharge instructions, follow-up and referral to community resources as appropriate
• Documents the evaluation and plan of care according to departmental guidelines
