The Legacy at Boca Raton Rehabilitation and Nursing Center in Boca Raton, FL is seeking a dedicated Physical Therapist (PT) to join our rehabilitation team. You will evaluate residents, develop individualized treatment plans, and provide high-quality therapy services to help residents achieve independence and functional mobility.

Qualifications

• Active Florida PT license required.
• Experience in skilled nursing or rehabilitation setting preferred.
• Strong clinical assessment and treatment planning skills.
• Excellent communication and teamwork abilities.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans.
• Provide therapeutic exercises, mobility and balance training, and pain management interventions.
• Monitor progress and adjust plans as needed.
• Collaborate with physicians, nursing staff, and other disciplines to optimize outcomes.
• Maintain accurate clinical documentation per regulations and facility policy.
• Educate residents and families about therapy goals and programs.
• Ensure compliance with all federal, state, and facility policies.
• Participate in care planning and discharge planning processes.
