Job summary
The Legacy at Boca Raton Rehabilitation and Nursing Center in Boca Raton, FL is seeking a dedicated Physical Therapist to join our rehabilitation team. The PT will evaluate residents, develop individualized treatment plans, and deliver therapy to improve independence and mobility.

Qualifications
- Active Florida Physical Therapist license in good standing.
- Experience in skilled nursing facility, rehabilitation center, or healthcare setting preferred.
- Strong clinical assessment and treatment planning skills.
- Excellent communication and interpersonal abilities.
- Ability to work collaboratively within an interdisciplinary team.
- Compassionate, resident-focused care approach.
Responsibilities
- Evaluate residents and develop individualized physical therapy treatment plans.
- Provide therapeutic exercises, mobility, balance training, and pain management interventions.
- Monitor progress and adjust treatment plans as needed.
- Collaborate with physicians, nursing staff, and interdisciplinary team for optimal outcomes.
- Maintain accurate clinical documentation in line with regulatory requirements.
- Educate residents and families about therapy programs and goals.
- Ensure compliance with federal, state and facility policies.
