Job summary

The Legacy at Boca Raton Rehabilitation and Nursing Center in Boca Raton, FL is seeking a dedicated Physical Therapist (PT) to join our rehabilitation team. You will evaluate residents, develop individualized treatment plans, and deliver high-quality therapy to improve independence and mobility.

Qualifications

• Active Florida PT license in good standing.
• Experience in skilled nursing or rehabilitation settings preferred.
• Strong clinical assessment and treatment planning skills.
• Excellent communication and interpersonal abilities.
• Ability to work collaboratively within an interdisciplinary team.
• Compassionate, resident-focused approach.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans
• Provide therapeutic exercises, mobility training, balance training, and pain management interventions
• Monitor resident progress and adjust treatment plans as needed
• Collaborate with physicians, nursing staff, and interdisciplinary team members to ensure optimal resident outcomes
• Maintain accurate and timely clinical documentation in accordance with regulatory and facility requirements
• Educate residents and families regarding therapy programs and treatment goals
• Ensure compliance with all federal, state, and facility policies and regulations
• Participate in care planning meetings and discharge planning processes
• Maintain a safe and supportive therapy environment
