Job summary

The Legacy at Boca Raton Rehabilitation and Nursing Center in Boca Raton, FL is seeking a dedicated Physical Therapist (PT) to join our rehabilitation team. Evaluations, individualized treatment plans, and high-quality therapy services will help residents achieve independence and functional mobility.

Qualifications

• Active Florida PT license in good standing.
• Experience in skilled nursing facility or rehabilitation setting preferred.
• Strong clinical assessment and treatment planning skills.
• Excellent communication and interpersonal abilities.
• Ability to work collaboratively within an interdisciplinary team.
• Compassionate and resident-focused approach to care.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans.
• Provide therapeutic exercises, mobility training, balance training, and pain management interventions.
• Monitor resident progress and adjust treatment plans as needed.
• Collaborate with physicians, nursing staff, and interdisciplinary team members to ensure optimal resident outcomes.
• Maintain accurate and timely clinical documentation in accordance with regulatory and facility requirements.
• Educate residents and families regarding therapy programs and treatment goals.
• Ensure compliance with all federal, state, and facility policies and regulations.
• Participate in care planning meetings and discharge planning processes.
• Maintain a safe and supportive therapy environment.
