The Legacy at Boca Raton Rehabilitation and Nursing Center is seeking a dedicated Physical Therapist (PT) to evaluate residents, create individualized treatment plans, and deliver high-quality therapy to improve mobility and independence. Qualifications

• Active Florida PT license in good standing.
• Experience in skilled nursing facility or rehab setting preferred.
• Strong communication and teamwork with residents and families.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans.
• Provide therapeutic exercises, mobility, balance, and pain management interventions.
• Monitor progress and adjust treatment plans as needed.
• Collaborate with physicians, nursing staff, and the interdisciplinary team.
• Maintain accurate clinical documentation.
