Job summary

Qualifications

• Licensed Physical Therapist in Georgia.
• All required certifications upon hire unless stated otherwise.
• BLS/CPR certifications are preferred.

Responsibilities

• Implements the physical therapy treatment plan.
• Selects appropriate treatment activities to progress patient toward goals.
• Revises goals/plan of care with patient/family input.
• Provides discharge instructions, follow-up and referrals to community resources as appropriate.
• Documents the evaluation and plan of care according to departmental guidelines.
