Responsibilities

• Assist clients in identifying recovery goals through 1-1 sessions and group treatments.
• Connect clients with resources and supports to meet treatment goals.
• Advocate for clients to access housing, transportation, and community services.
• Support clients in building social connections and vocational readiness.
• Maintain accurate documentation and progress reports for courts/referrals as needed.
• Collaborate with the behavioral health team to coordinate care.
