Qualifications

• Master's degree in Social Work required.

Responsibilities

• Conduct biopsychosocial assessments and develop care plans.
• Provide individual, family, or group therapy and discharge planning.
• Coordinate with interdisciplinary team and refer to authorities as needed.
• Document assessments and interventions per department standards.
• Monitor quality and manage population outcomes; liaise with payers.
• Stay aware of community resources and connect patients/families.
