Job summary

Endeavors is seeking an Intake Coordinator/Outreach Specialist to support the CBP Workforce Wellness Program. You will coordinate community outreach, foster partnerships, and engage participants to access wellness resources for CBP employees and families.

The role supports outreach events, documentation, and collaboration with program managers to improve referrals and service delivery while delivering exceptional customer service.

Qualifications

• Bachelor’s degree preferred, or equivalent experience.
• Proven ability to work within an interdisciplinary team.
• Experience in outreach and customer service environments.

Responsibilities

• Conduct eligibility screenings for individuals and families seeking program services.
• Develop relationships with community organizations and referral partners to raise awareness.
• Provide information on program services and connect individuals to internal or community resources.
• Establish and maintain participant records with accurate intake documentation.
• Coordinate referrals and connect individuals to wellness, case management, and other resources.
• Monitor engagement and address barriers to participation.
• Prepare reports on outreach activities, referrals, and participant engagement.
• Represent the organization at community events and outreach activities.

Education
